This is the original S-77 form that must be submitted to the Watchtower Bible and Tract Society when a Jehovah's Witness either disassociates himself or is disfellowshipped from the congregation. Information required on the form to be filled out by elders are congregational data and personal information about person and reasons for disfellowshipping. This is an intra-organizational form for Jehovah's Witness elders and Watchtower personnel ONLY. It is not readily available to JW's unless they are elders.
